Well, it's Manganiello's face PhotoShopped onto someone else's body, at least.  A Google Images search on Manganiello's name shows dozens of photos of him shirtless, and his chest is smooth in all of them, like in photo #1 below, with only one exception.  The only Google Images photo of him with a hairy chest is a tiny one with him in a jockstrap, so it's also probably a fake.  His forearms don't even seem to be very hairy, and he doesn't appear to have much of a happy trail, either, vs. the hairy stomach in the fake above.  

But the image above appears to be a pretty damned sophisticated PhotoShop job, though -- it even includes a necklace that appears to be similar to the one Manganiello is wearing in some of his Google Images shots.  Unfortunately, the photo isn't large enough to examine for any of the usual tell-tale signs of manipulation, like discoloration in the skin tone where the alteration was done.  

#2 below may be someone's attempt to contrast Manganiello's smooth chest with that of his "Magic Mike" costar, Adam Rodriguez, but the Rodriguez image is a fake, too. Shirtless photos of him from Google Images show that he doesn't have a hairy chest, either.  The body on the right in #2 is almost certainly the original of the one used as the basis for the image above, though.  And from comparing that photo to the one above, it seems that whoever did the fake image even added the cock to the body, too, as well as Manganiello's head.
